An assessment for a neurologist, a letter to the GP, feedback for a child’s parents, a certificate for an employer. Theramate writes these documents from the patient file, with the template, the recipient and the sessions you choose. Psychologists, psychotherapists, psychiatrists, speech therapists, psychomotor therapists, occupational therapists, sexologists and coaches find templates specific to their profession.
Creating a report in two steps
In the patient file, the « Clinical Data » card leads to « Your clinical reports ». The « New report » button opens the library.
- You choose the template, among Theramate’s templates or your own.
- You choose « Who is it for? » and the « Report sources », then click « Generate the report ».
The report is written in the background. You carry on with your work, and a notification tells you when it is ready.
Theramate’s 32 templates

Choosing the recipient
With « Who is it for? », tone, voice and what is said adapt to the reader. Each template marks the « Recommended » choice.
| Recipient | What the report says |
|---|---|
| A clinician | From one practitioner to another, what serves their care |
| The patient or their family | Addressed to them, in plain language, no jargon |
| A non-clinical third party | Only what is needed |
| My file | A dense working note, with your hypotheses |
| Traceable summary | Every item dated and linked to its session |
- « A clinician » narrows down to « Mental health », « Physician » or « Other clinician », and the letter is written for that reader, from the psychiatrist to the GP or the physiotherapist.
- « The patient or their family » narrows down to « The patient » or « Their family ». The text speaks to them directly, for example to give the results of an assessment to a child’s parents.
- « A non-clinical third party » covers the school, the employer or the insurer. The report contains no diagnosis, no hypothesis, no intimate detail and no risk assessment.
- « My file » keeps your working hypotheses, to prepare a session or an intervision.
- « Traceable summary » dates every item and links it to its session.
The functional assessment for the GP
The « Functional assessment » follows the format agreed with the GP. It covers the complaints, the request and goals, medication, the therapeutic proposals and progress. It can be written from the first session, and it is addressed to the physician by default.
Choosing the sessions to cover
The « Report sources » set the period the report covers.
| Source | What the report covers |
|---|---|
| Since the last generation | The sessions since the last report from the same template |
| Latest consultations | The number of consultations you choose |
| Custom period | The sessions between two dates |
| Whole file | All the patient’s consultations |
For a quarterly progress review, « Since the last generation » covers exactly what has happened since the previous one.
Reviewing and refining
The report only contains what is in the file, and it stays yours. Theramate never rewrites it on its own. Your signature and your stamp are added at export.
- You edit the text freely, as in a word processor.
- On a selected passage, the assistant offers « Condense », « Expand », « Rephrase » or « Simplify ». You can also « Target this section » or « Write a custom instruction ».
- Its suggestions appear in colour, and you accept or reject them.
- Every version is kept and can be reread in read-only mode.
- When new consultations come in, the report shows « New consultations since this report », and « Update » writes a new version.
Exporting and sending
« Export » produces a PDF with your letterhead, your signature and your stamp, which you save once in your profile.
- « Anonymize the document » replaces names, exact dates and identifying details, for supervision, a letter to a colleague or a case study. You choose « Respectful initials » (“Mrs D., 42”) or « Generic wording » (“the patient, 42”), and the original document is kept.
- Sending goes through a temporary link, valid for 7, 30 or 90 days. With « Require access code », on by default, the recipient opens the link with a four-digit code that you give them through another channel. The PDF is never sent as an attachment.
The file is hosted and operated in the European Union, encrypted with a key specific to each practitioner. The details are on the Security page.
Your own templates
Your assessments keep their usual format. The « My templates » tab offers two starting points.
- « Import an existing template », from your blank report as a PDF, a Word file or an image. A PDF keeps its layout, and Theramate fills in its fields one by one.
- « Create with the assistant », by describing the sections you want, for example a psychomotor assessment with history, tests, observations and therapy plan.
Your template can then be reused for all your patients, with the same choice of recipient and sessions. The details are in Theramate adapts to your practice, from CBT to psychomotor therapy.
Administrative forms
Forms from a health insurer, an insurance company or an administration are filled in from the same file. You start from the « Catalogue » or a « New form », from your PDF or Word document, and Theramate detects its fields.
- « Fill with AI » completes the contact details, dates, amounts, your signature and your stamp.
- « Fill manually » lets you enter each field.
The report and the session summary
The summary is written each time you close a session and stays in the file. The report brings several sessions together for a specific reader. The two complement each other.
The session summary is described in Everything Theramate prepares when you close a session. The questionnaires a report can include are in Longitudinal follow-up of your patients, with questionnaires and self-observations.
The criteria for choosing practice software are gathered in How to choose software for your therapy practice in 2026.
- Thirty-two templates, sixteen shared by every profession and sixteen specific to one.
- « Who is it for? » adapts the tone and content to a clinician, the patient, a non-clinical third party or your own file.
- You choose the sessions the report covers, from the last generation to the whole file.
- The text stays yours. You refine it passage by passage and every version is kept.
- The export carries your letterhead, your signature and your stamp, and goes out by a link protected by a code.
- Your own templates and your administrative forms are filled in from the same file.
