After a session, Theramate puts your file in order for you. It writes the session report, organises what was said, updates the clinical file, fills in your own follow-up templates and prepares the tasks you mentioned. You don’t wait and you don’t start anything. You close the session and move on to the next patient.

Close the session and move on

Click « Complete Consultation ». There is no waiting screen. You can open the next session, answer a message or close the app.

The report and the file update run in the background, within a few minutes, from your notes, your handwritten notes and the recording if you made one. When you open the file again, everything is in place.

The session report

The report can be read in one to two minutes. It always follows the same three sections.

  • Session summary, in short sentences.
  • Follow-up, with the patient’s commitments and the next appointment.
  • Therapist actions, each with what needs doing and by when.
Session report · individual sessionWritten on closing
Summary
Argument with her sister on Saturday about the inheritance, came home "drained". Sleep still disrupted, waking at 4 am, 3 nights out of 7.
Follow-up
Note night-time awakenings in the diary. Next appointment Tuesday at 6 pm.
Actions
Prepare the letter for the GP before Friday.

It stays faithful to the patient’s words.

The report is written in your working language, whatever language was spoken in the session.

Your own reports, filled in after every session

Every therapist notes different things. With « My custom follow-up », you build your own after-session report, and Theramate fills it in automatically after every session with that patient.

You describe what you want to note, the way you would explain it to a colleague. The assistant builds the template with you, section by section. Each section can be a list, quotes, a paragraph or a table. It can stick to what was said in the session, or offer a few hypotheses, each tied to a specific moment of the session.

A few examples, depending on your therapeutic approach.

  • Automatic thoughts and the associated emotion.
  • Exposure tracking, situation by situation.
  • Dreams reported and their associations.
  • Bodily sensations mentioned, pain and tension.
  • Your own feelings in the session, countertransference.

A preview fills in the template with the patient’s last session before you switch it on. After that, every session fills it in under the « My follow-up » tab of the consultation. Keep it in your templates to reuse it with other patients.

A file that builds itself session after session

The report is connected to the rest of the file. Each time you close a session, it feeds four views of the file that build up over the course of therapy.

  • The session organised by theme, in five tabs.
  • The clinical file, which keeps the essentials about the patient on one page.
  • Clinical trends, which track sixteen markers over time.
  • The therapy journey, which lets you reread a whole course of therapy in a few minutes.

After a few months, you have an orderly file without having copied anything out.

The session organised by theme

Under the report, five tabs look at the session from a specific clinical angle.

Therapeutic

Changes in the therapy, goals, strategies introduced or adjusted, medication, progress and resistance. For example “Sertraline ↑ 50 → 75 mg (morning anxiety)”.

People

Every person mentioned, and the pets that matter to the patient, with their role and a short description. For example “The patient’s dog, a source of comfort”. This is where you find the patient’s circle and family dynamics.

Trauma

Traumatic events with their period, current symptoms, triggers and avoidance. For example “Sound of braking → immediate flashbacks, on alert”.

Patterns

Recurring cycles, avoidance and automatic behaviours. For example “Stress → withdrawal → guilt”.

Resources

The patient’s resources, coping strategies, strengths and soothing activities. For example “Daily walk, unwinding, ‘it clears my head’”.

The clinical file, kept up to date

The clinical file fits on one page and always follows the same outline.

  • Life context and resources
  • Reason for referral and therapeutic goals
  • History, psychiatric, medical and life events
  • Problem list, symptoms, relationship difficulties, functioning, patterns
  • Therapeutic landmarks, phase of therapy, points of vigilance and levers

It is updated after each session, and you can see at a glance what has changed.

You will find it in « Clinical Data », among the « Auto-maintained reports ».

After each session, Theramate estimates sixteen clinical markers and adds them to their curves. They are grouped into four families.

  • Safety, with suicidal ideation, hope, crisis risk and safety plan.
  • Symptoms, with depression, anxiety, daily functioning and sleep.
  • Therapeutic process, with alliance, engagement, momentum and insight.
  • Between sessions, with coping strategies, social connection, stress and life events.

They are estimates drawn from your sessions, with no diagnostic value, showing progress over a week, a month, three months or a year.

The therapy journey

Each session gets a short paragraph giving its main issue, and four to six keywords. In the patient’s history, the « Therapy journey » view lines up these paragraphs so you can reread a whole course of therapy in a few minutes.

Tasks picked up from the session

Every action noted in the report becomes a patient task, without you having to type it in. Theramate then sorts them into two groups.

Tasks Theramate can prepare

When a task can be done from the app, Theramate recognises what needs preparing and for whom.

  • “Letter for the GP before Friday” becomes a document to write for the doctor, with its deadline.
  • “Send the DASS-21 before the assessment” becomes a DASS-21 ready to send. You review it and can pick another one before sending.
  • “Repeat the PCL-5 in a month” becomes a PCL-5 ready to go when the time comes.
  • “Suggest a sleep diary” becomes a self-observation to create for the patient.

« Prepare a draft » writes the document from the file. You review it, adjust it and send it with « Approve and share ». Templates and recipients are described in Theramate writes your reports and letters from the patient file.

Administrative details kept up to date

During the session, the patient mentions a new GP, a new health insurer or a new address. Theramate picks up the change and files it in an « Update … in the file » task. With « Review the changes », you see for each field the current value, the new value, which you can correct, and the sentence from the session that supports it. You untick what you set aside and save. Nothing goes into the file without your approval.

Tasks left for you to do

A call to make, a case to bring to supervision or an official form to fill in stay as reminders in your tasks. You tick them off when they are done.

Nothing is prepared or sent without your click.

You stay in control

The report can be edited like any text. To check a sentence, select it and click « Trace to source ». Theramate finds the exact passage in the transcript or in your notes.

If you correct your notes, the report, the session organised by theme and the therapy journey are redone with the correction.

Transcription is described in the article Theramate transcribes your sessions word for word, in 60 languages. The patient files page shows the full file.

The criteria for choosing practice software are gathered in How to choose software for your therapy practice in 2026.
